Housing & Transition Services

National Foster Youth Institute – Housing Resources NFYI shares national and state-level housing programs designed to help former foster youth find safe, affordable housing after leaving care. This includes information about public housing, transitional programs, host homes, and emergency housing support. https://www.nfyi.org/issues/housing/
360 Youth Services Offers a structured program designed to help youth make improvements towards independence. 360 Youth Services offers a safe and affirming place to live along with other supports such as counseling services and life skills classes. To talk to a member of the 360 Housing Team, call 331-280-1985 or send an email to housing@360youthservices.org.
The National Resource Center for Youth Services (NRCYS) NRCYS offers resources and training programs focused on helping youth successfully transition out of foster care. Topics include housing stability, education, employment, and building permanent connections. The site includes tools for both youth and the professionals supporting them. https://nrcys.ou.edu/
DCFS – Extended Foster Care (up to age 21) Youth in Illinois can remain in foster care beyond age 18 through Extended Foster Care services. This allows continued access to housing, education, medical, and case management services while youth transition to adulthood. The goal is to ensure a smoother, more supported path to independence. https://dcfs.illinois.gov/youth/extended-foster-care.html
DCFS – Transitional Living Program (TLP) DCFS Transitional Living Programs offer structured, supervised housing for older youth in care (ages 17–21) who are preparing to live independently. Youth in TLPs receive support with life skills, education, employment, budgeting, and personal development in a safe, stable environment. https://dcfs.illinois.gov/youth/transitional-living-program.html
DCFS – Youth Housing Assistance Program (YHAP) The Youth Housing Assistance Program provides eligible youth aging out of foster care with financial support and services to secure stable housing. YHAP can assist with security deposits, rent, utilities, and essential household items. Caseworkers also help youth develop independent living skills and create a plan for long-term stability. https://dcfs.illinois.gov/youth/yhap.html
